The New Rules (as of July 17, 2006) define the minimum items needed for a locate request. They are: 1. The name and contact information of the excavator. 2. A description and the purpose of the type of work to be done. 3. The name of the person for whom the work is being done. 4. Whether or not the excavation site is pre-marked in white. 5. An accurate physical description of the location and size of the excavation site. Reference to a plat of a subdivision shall not by itself be sufficient description. 6. Driving instruction to a rural excavation site. 7. Spotting instructions. 8. Any appropriate remarks regarding access to or hazards at the site.
